Corrections, Duplicates and the Paper Trail

Requests to change a record are routine. What separates a tidy office from a chaotic one is how the change is recorded.

Names get misspelled, dates of birth arrive transposed, a subject is entered against the wrong candidate. A centre that treats each of these as an emergency will handle them inconsistently; one that treats them as a standing process will handle a hundred a year without incident.

The process needs four things: who asked, what evidence they produced, what changed, and who approved it. Recorded against the student, those four turn a correction from an act of trust into a fact you can produce later. Recorded nowhere, the same correction is indistinguishable from an error.

Re-issues follow the same logic. Note that a document was reprinted, when, and why, and keep the serial. The cost is a few seconds at the counter, and it is repaid the first time somebody presents a document you have no memory of issuing.
